​NOTICE
By signing this document, you are giving up certain legal rights, including the right to sue. This waiver and release is intended to be valid and binding under California law and consistent with the rules and policies of National Pop Warner Little Scholars, Inc. ("Pop Warner").

1. ASSUMPTION OF RISK
I, the undersigned parent or legal guardian, acknowledge that participation in football, cheer, or any youth camp and/or clinic event involves inherent risks, including but not limited to, serious bodily injury, permanent disability, paralysis, illness (including communicable diseases such as COVID-19), and death. I understand that these risks may result from the actions or inactions of participants, volunteers, coaches, spectators, equipment, facilities, or environmental conditions. I voluntarily assume all such risks on behalf of my child.

2. RELEASE OF LIABILITY
In consideration of my child being allowed to participate in camp and/or clinic activities hosted by Del Rancho Pop Warner, I hereby release, waive, discharge, and covenant not to sue Del Rancho Pop Warner, Pop Warner Little Scholars, Inc., its regional and national affiliates, officers, directors, agents, coaches, volunteers, sponsors, insurers, and any representatives (collectively, the "Released Parties") from any and all claims or liabilities, present or future, for personal injury, property damage, wrongful death, or any other cause arising from participation in Pop Warner activities, whether caused by negligence or otherwise.

3. INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, and expenses, including reasonable attorneys' fees, arising out of or related to my child's participation in the camp and/or clinic event.

4. MEDICAL AUTHORIZATION
I authorize Del Rancho Pop Warner and its representatives to obtain emergency medical care for my child if deemed necessary. I agree to be financially responsible for any medical treatment provided.

5. COMPLIANCE WITH RULES
I acknowledge that my child must comply with all applicable rules, regulations, and policies of Del Rancho Pop Warner and National Pop Warner, including those related to conduct, safety, and health protocols.

6. MEDIA RELEASE (Optional)
I grant permission to Del Rancho Pop Warner to use my child's image, name, and likeness in promotional materials, including but not limited to photographs, videos, and social media. I understand that this use is without compensation.

7. SEVERABILITY
If any provision of this Agreement is found to be invalid or unenforceable under applicable law, the remaining provisions shall continue in full force and effect.
